Person-centred care plans are prepared in consultation with the residents, their relative / representative and all other appropriate external social or healthcare professionals wherever possible.
This enables us to develop an all-round comprehensive view of the resident’s physical, spiritual and emotional needs, which enables the Home’s management to produce a comprehensive assessment of all aspects of the resident’s health, personal and social care requirements.
Each individual resident has a person-centred plan of care, which covers every aspect of their daily living. These are reviewed monthly by the management or more often should the resident’s requirements change, in consultation with the resident, their relative/representative and all other appropriate external social or healthcare professionals wherever possible. Details of reviews are documented and recommendations acted upon.
